Please also submit a note of your request on your organisation letterhead paper and upload to us together with proof of either your charitable status or your affiliation to the organisation you represent.

Please submit the completed form or otherwise deliver it to Bramshall Farm at Bramshall Road, Uttoxeter ST14 5BE. Please include a formal letter of request on your organisations letterhead and include documentation of your charitable status or affiliation with a recognised institution.
